The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves.

The Colorado Coalition for the Homeless seeks a Clinical Quality Improvement (CQI) & Informatics Specialist to drive ongoing quality improvement efforts. The informatics specialist will collaborate closely with clinical staff to drive QI projects utilizing data collected across multiple sources and with extensive knowledge of industry best practices. This position is located at 2130 Stout St., Denver, Colorado 80205, and reports to the Vice President of Integrated Health Services.

NOTE: In order to ensure that our consumers receive the best possible care the candidate chosen to fill this position will be required to complete our internal credentialing and privileging process prior to a start date. The credentialing and privileging timeline is dependent on the selected candidate's submission of documents necessary for clearance. Please note that a start date will be scheduled once credentialing and privileging is complete.

Essential Job Functions

  1. Work with clinicians, the IT department and CCH leadership to develop processes which focus on continually improving the care delivered to patients.
  2. Understands the collection, management, and structure of data across the organization to ensure all available, relevant data is utilized in analysis and development of improvement efforts.
  3. Analyze and interpret data to identify micro and macro-level gaps in care.
  4. Assess and implement improvements to workflow and EHR documentation challenges which impact the quality of clinical care delivered.
  5. Develops and maintain extensive knowledge of best practices within the field of CQI and clinical informatics to increase clinical and operational efficiency.
  6. Brings innovative ideas, processes, and/or procedures for enhancements, additions, and modifications to improve the EHR and integrated systems, including system customizations, data integration, and workflow enhancements.
  7. Collaborate with stakeholders to design and implement systems focused on patient safety, delivery of evidence based clinical care and regulatory compliance.
  8. Identify metrics and monitoring systems to achieve the desired state of systematic and consistent performance.
  9. Facilitates on-going internal and external reporting and communications regarding the status of outcomes related to continuous improvement and performance related to clinical quality measures.
  10. Evaluate and communicate on the effectiveness of quality improvement efforts and identify areas for opportunities.
  11. Work collaboratively with multiple internal and external partners.
  12. Perform other duties as assigned.

Qualifications Summary

  1. Minimum of 3-5 years' experience in healthcare with progressive levels of leadership required.
  2. Background within a clinical setting such as a nurse, nurse practitioner, or physician assistant with a strong knowledge of clinical workflow is preferred.
  3. Experience in operational and performance improvement in clinical settings required.
  4. Experience with LEAN Process Improvement, Six Sigma, Balanced Scorecard, or similar methodology preferred.
