Required Qualifications:

• Undergraduate Degree or 3 years of experience working within the community health setting in a health care role (or experience as mandated by the state contract)
• 1+ year of experience in Maternal Health and Pediatrics
• Experience in case management or care coordination
• Experience working with MS Word, Excel and Outlook
• Reside within Larimer County
• Ability to travel in assigned region to visit Medicaid members in their homes and/or other settings, including community centers, hospitals or providers' offices
• If you need to enter a work site for any reason, you will be required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or similar UnitedHealth Group-approved symptom screener. When in a UnitedHealth Group building, employees are required to wear a mask in common areas. In addition, employees must comply with any state and local masking orders

Preferred Qualifications:

• LPN / LVN, CNA, licensed social worker and/or behavioral health or clinical degree
• Background in managing populations with behavioral health needs
• Experience with electronic charting
• Prior field-based work experience

Colorado Residents Only: The hourly range for Colorado residents is $20.77 to $36.88.
