Job Overview

Ensures safe, appropriate, and economical use of drugs through the application of specialized skills and knowledge in pediatric patient care.

The clinical pharmacy specialist is responsible for providing clinical pharmacy services for the CICU and CPCU areas of the Heart Institute. The pharmacist will rotate between the CICU and CPCU.

Responsibilities

POPULATION SPECIFIC CARE

  • a.) Neonate - <30 days
  • b.) Infancy - >30 days to 1yr
  • c.) Toddlers - >1yr to 3yrs
  • d.) Pre-Schoolers - >3yrs to 5yrs
  • e.) School age - >5yrs to 13yrs
  • f.) Adolescent - >13yrs to 18yrs
  • g.) Adult - >18yrs to 65yrs

ESSENTIAL FUNCTIONS

  • Attend daily multi-disciplinary rounds
  • Monitor and evaluate patient specific pharmacotherapeutic regimens
  • Ensure safe, age appropriate, cost effective therapies according to the established guidelines, policies/procedures, protocols, etc.
  • Participate in the development of clinical care pathways, protocols, order sets, etc.
  • Proactively recognize the need for improvement and participate in quality improvement/process improvement initiatives that optimize the quality of pharmacy services to the general medicine patients
  • Oversee a seamless transition of pharmacy services between inpatient and outpatient setting
  • Serve as a clinical pharmacy resource to the general medicine providers and other healthcare professionals
  • Medication reconciliation and discharge medication teaching
  • Facilitate drug information requests from patients, families and healthcare professionals
  • Serve as a member of hospital committees
  • Establish collaborative relationships with the general medicine providers focusing on medication therapy monitoring
  • Promote education and training of pharmacy students, pharmacy residents and other healthcare professionals
  • Submitting and presenting (if accepted) abstracts/posters at annual meetings
  • Submitting articles for publication to share Children's Hospital Colorado general medicine practices/experience
  • Designs and leads pharmaceutical outcomes research. Identifies opportunities for and participates in drug utilization reviews and research. Identifies opportunities for publishing case reports, unique therapeutic interventions, development of practice standards, and other medication related pharmacy topics.
  • Demonstrates sustained record of contributing to the pharmacy practice knowledge through:
    • Publications in professionals journals or books
    • Posters/presentations at professional meetings (regional/national level)
    • Review of book chapters and manuscripts submitted for publication
  • Participates in the education of pharmacy staff, nursing staff, medical staff, pharmacy residents and students, other healthcare allied professionals and patients and families. Serves as a preceptor and mentor for pharmacy residents and students.
  • Participates on a hospital committee(s)
  • Provides drug information services by responding to questions regarding medication compatibilities, adverse drug events, regimens, dosing, interactions, storage and disposal, compounding, therapeutic uses of medications and pharmaceutical supplies in a timely manner; using a systematic approach to effectively searching, retrieving, and evaluating the literature and appropriately communicating and applying the information to the patient care situation; and assisting with the development, maintenance, and implementation of hospital and department policy and procedures related to medication use, storage, restrictions, and administration.
  • Sustains and implements quality and safety initiatives and practices in the department and throughout the hospital. Reports adverse drug events and medication variances using the hospital variance reporting program (QSRS). Demonstrates sustained record of identifying and leading quality improvement/assurance initiatives.
  • Sustains clinical pharmacy services throughout the organization by providing pharmaceutical care for pediatric patients of all age ranges through participation in multi-disciplinary team rounds and/or care, emergency response, education, guidance, interpretation and analysis of medication related therapeutic issues; documenting pharmaceutical interventions according to departmental standards; performing therapeutic drug monitoring and disease outcome analysis; and timely verification of medication orders in an electronic health record system.
  • Participate in general pharmacy operations throughout the organization through appropriate medication preparation, dispensing, and inventory control. Provides oversight to pharmacy technicians, interns, residents and students as needed.

  • SUPERVISION RECEIVED: Performs work with minimal supervisory oversight. Work assignment is generally unstructured and employee is responsible for organizing complex, varied, and simultaneous coordination of several functions, programs, or projects in various stages of completion.
Qualifications
  • EDUCATION: Doctor of Pharmacy or Bachelor of Science in Pharmacy degree
  • CERTIFICATION: Board Certification - required (must obtain within 18 months)
  • EXPERIENCE:
    • PGY1 Residency - required
    • PGY2 Residency (pediatrics) or 3 years of inpatient pediatric experience
    • Previous parenteral nutrition experience - preferred
  • LICENSURE: Colorado pharmacist license
  • EQUIVALENCY: Three (3) years of pharmacy work experience in area of specialty accepted in lieu of 2nd year of residency.
