Description:
The Program Manager of Population Management and Integrated Care is responsible for designing and leading programs associated with strategic business issues and opportunities. The Program Manager of Population Management and Integrated Care may manage specific aspects of one or more programs, or multiple aspects of a single program within the organization. Criteria for moving from one level to the next includes greater skills and knowledge, built on experience (track record) and job assignments. This job family is intended for use in non-clinical/non-hospital settings.

Essential Responsibilities:
  • Works strategically and collaboratively with members of the Executive, Physician, IT and Operations team to develop a strategic vision, product roadmap and execution plan to increase member health and wellness penetration, outreach and adoption to successfully grow the new program suite.
  • Responsible for the optimization and monitoring of software systems, virtual care and venue alternatives utilized by the department and market to ensure efficient operations within the department and transactional systems outside of the department.
  • Develops, directs, implements, and evaluates new and existing programs targeting varying member populations to enhance company brand and results.
  • Influences staff, physicians, and leadership with knowledge regarding the value and appropriate use of this program.
  • Leads, monitors and supports quality improvement projects tied to Population Health and ICC. Provides multi-disciplinary case consultation as needed. Collaborates with researchers and other program support to demonstrate value while streamlining the care experience.
  • Ensures compliance with contractual, regulatory, and accreditation requirements.
  • Demonstrates leadership potential and aptitude for working in the technology and healthcare fields and demonstrates knowledge of emerging trends in the industry.
  • Collaborates with leaders of other care/case management programs at Kaiser Permanente to ensure a well-integrated and seamless process and system for staff and providers wishing to access this model of care for their patients. Supervises assigned staff to include interviewing, selecting, training, motivating, evaluating, counseling, disciplining and terminating in compliance with EEO/AA goals and personnel policies of the organization.
  • Ability to work with data and reporting tools to research, organize, and summarize findings. Assesses and develops recommendations for new or revised nursing policies, procedures, and guidelines. Networks within the organization and the community to identify resources and information critical to the care managers as they work to achieve the highest quality of care for our patients.
  • High technical and functional aptitude for problem solving and process flow management.
  • Basic Qualifications:
    Experience
  • Minimum five (5) years of program management experience in a related field, diversified healthcare or health insurance company.
  • Minimum three (3) years of administrative experience in a clinical environment (e.g., faculty practice plan, clinical department of an academic medical center, medical group practice, or clinical operations department of a managed care organization) supporting physicians, process improvement and operational initiatives.
  • Education
  • Bachelor's degree in business, healthcare or a related field required.

  • Additional Requirements:
  • Requires full knowledge of own area of functional responsibility and working knowledge of another function.
  • Knowledge of utilization management principles and health plan managed care, HMO, PPO and POS Insurance.
  • Ability to lead cross functional teams, mentor and support the development of associates.
  • Experience in government sponsored managed care programs.
  • Experience with Microsoft Office tools, or similar.
  • Good organizational and communication skills.
  • Experience with Epic or similar EMR; or managed care software applications.
  • Self-motivated and high level of personal initiative.
  • Strong interpersonal skills to work as part of a team on large projects.
  • Ability to manage time and responsibilities in a fast paced, fluid environment.
  • High technical and functional aptitude for problem solving and process flow management.

  • Preferred Qualifications:
  • Master's degree in public health or healthcare administration preferred.
